Accessing Medical Records
Athens-Limestone Hospital offers an online patient record request tool for the following individuals:
- Patients requesting their own records
- Parents requesting records for minor patients
- Caregivers acting on behalf of a patient (i.e. Power of Attorney)
A copy of the Power of Attorney will need to be attached while completing this request using our online tool. If the patient is deceased, please make sure to attach a copy of the death certificate. The online tool verifies your identity using a valid photo ID, which can be submitted via smartphone or webcam.
How to Request Medical Records
Patients may request records online, by mail or through their physician.
Patients (or legal representatives) will be required to complete the appropriate authorization forms and provide valid identification. If you experience issues with the online request tool, please call 256-233-9464.
Request by Mail
If you are unable to use the online tool, you may request records by mail. To request records by mail:
- Complete the Authorization to Disclose Health Information Form
- Include a copy of a valid photo ID
- Provide complete patient and request details

Processing Times & Fees
Once Medical Records receives your completed request:
- Standard processing: Approximately 7–10 business days
- Paper copies (mail): $0.12 per page
- Electronic delivery (PDF): Flat $6.50
There is no charge for:
- Records sent directly to a physician for continuing care
- Veterans or active-duty military personnel with valid ID
Patient Portal Access
A summary of your inpatient stay may be available through the Patient Portal.
If you do not currently have a login, you will need your discharge paperwork to sign in. Records not included in the portal must be requested through the Medical Records Department.
Requests From Physician Offices
If records are needed for an appointment within 48–72 hours, your physician may request them by fax.

There is no charge when records are sent directly to a physician or facility for treatment.

Radiologic Images
(CD or Film)
To request radiologic CD images and/or film, contact Imaging Services at 256-216-9696. You can also make a request via fax at 256-233-9573.
Itemized Statements / Statement of Charges
For billing-related statements, contact Patient Accounting at 256-233-9124.
Amendment Requests
You have the right to request corrections to your medical record if you believe information is incorrect or incomplete. Submit requests to the Medical Records Department. If a request is denied, you may submit a statement of disagreement that will be included in future disclosures.
