Accessing Medical Records
Decatur Morgan Hospital offers an online patient record request tool for the following individuals to request patient records:
- Patients requesting their own records
- Parents of minor patients requesting records
- Caregivers acting on behalf of a patient (i.e. Power of Attorney)
A copy of the Power of Attorney must be attached when completing the request. If the patient is deceased, a copy of the death certificate must also be provided. The online tool verifies identity using a valid photo ID (such as a driver’s license, passport or other government-issued identification), which can be uploaded via smartphone or webcam.
How to Request Medical Records
Patients may request copies of medical records by phone, by mail or online. Patients (or legal representatives) will be required to complete the appropriate authorization forms and submit a copy of a valid photo ID.

In-person requests may be made Monday through Friday, 8 a.m. – 4:30 p.m.

- Complete the Authorization to Disclose Health Information Form
- Include a copy of a valid photo ID

Processing Times
Once Medical Records receives your completed request:
- Electronic copies: Approximately 3 business days
- Paper copies: Approximately 5–7 business days
Processing times begin once all required documentation has been received.
Online requests may take approximately 7–10 business days to process.
Patient Portal Access
A summary of your inpatient stay may be available through the Patient Portal.
If you do not currently have a login, you will need your discharge paperwork to sign in. Records not included in the portal must be requested through the Medical Records Department.
Requests From Physician Offices
Physician offices or other health care providers may request medical records by faxing a request on official office letterhead to:

Radiologic Images

To request radiologic CD images and/or film, contact Medical Records at 256-973-2691.
Requesting Records for Someone Else
If the patient is under the age of 16, physically or mentally incapacitated, or deceased, a legally designated representative may complete the required forms and submit the request on behalf of the patient.
