Requesting Your Medical Records
Patients or authorized representatives may request copies of medical records for personal use, continuing care, insurance purposes or other needs.
Requests typically require:
- A completed authorization form
- A valid photo ID
- Additional documentation if requesting records on behalf of another individual
Processing times and available request methods may vary by hospital.
How to Submit a Medical Records Request
Depending on the hospital, medical records requests may be submitted:
- Through an online request form or patient portal
- By mail
- In person (where available)
