HH Health is committed to protecting patient privacy while providing access to medical records in accordance with state and federal regulations. Because medical records are maintained by each hospital, requests must be submitted to the hospital where care was received.

Requesting Your Medical Records

Patients or authorized representatives may request copies of medical records for personal use, continuing care, insurance purposes or other needs.

Requests typically require:

  • A completed authorization form
  • A valid photo ID
  • Additional documentation if requesting records on behalf of another individual

Processing times and available request methods may vary by hospital.

How to Submit a Medical Records Request

Depending on the hospital, medical records requests may be submitted:

  • Through an online request form or patient portal
  • By mail
  • In person (where available)
