HH Health offers access to home health services that bring skilled medical care, therapy and support directly to patients where they’re most comfortable. Through trusted hospital-based programs and community partners, home health allows patients to recover from illness, injury or surgery while maintaining independence and continuity of care.
Home health services are typically provided under the supervision of a physician and may be recommended following a hospital stay or to help manage chronic conditions.
What Is Home Health?
Home health care includes a wide range of medical and therapeutic services delivered in a patient’s home. It is often more convenient and cost-effective than inpatient or skilled nursing facility care, while remaining just as clinically effective for many conditions.
Home health is an extension of your doctor’s care and may include skilled nursing, therapy services, education and ongoing monitoring tailored to your individual needs.
Services Commonly Provided
Services vary by location and provider, but may include:
- Skilled nursing care
- Medication management
- Wound care
- Chronic disease management (including diabetes, heart failure, hypertension and COPD)
- Physical, occupational and speech therapy
- Home safety and fall risk assessments
- Patient and family education
- Infusion therapy (select locations)
- Nursing aide assistance
Your care plan is developed in coordination with your physician and adjusted as your recovery progresses.
Who May Qualify for Home Health?
Home health services are typically available to patients who:
- Are under the care of a physician
- Require skilled medical or therapy services on an intermittent basis
- Are considered homebound (meaning leaving home requires considerable effort or assistance)
You do not need to be bedridden to qualify. Eligibility and coverage are reviewed on a case-by-case basis, often during hospital discharge planning or physician referral.
How to Access Home Health Services
A physician referral is required to begin home health services. You can start the process through:
- Your doctor or physician’s office
- A hospital case manager or discharge planner
- A medical social worker
